Silicone Implants

Breast augmentation continues to increase in popularity each year, and implant manufacturers have been developing a variety of new breast implant options to meet this growing demand. In 1992, the FDA removed silicone implants from the list of approved materials, making saline implants the only viable option for the next 14 years.

During that time, a several studies were conducted to determine whether or not silicone implants posed a danger to women. After years of research, the FDA concluded that silicone implants were in fact safe, and in 2006, they were once again approved for use in breast augmentation procedures.

Over the past four years, silicone implants have become the preferred choice of a growing number of plastic surgeons due to their natural looking and feeling results. Recent advances in silicone implant technology have led to the development of cohesive gel, or "gummy bear" implants. While cohesive gel implants are still in the clinical testing phase in the United States, it is likely that once they receive FDA approval, they will become a popular option among women seeking breast augmentation.

What are Cohesive Gel Implants?

Cohesive gel implants are made from a firm, yet soft silicone gel material. Additional crosslinker material is added to the silicone gel, resulting in this increase in firmness. As a result, cohesive gel implants maintain their shape better than traditional silicone implants.

This is an important development in implant technology because in the event of a rupture, cohesive gel implants are much less likely to leak silicone into the blood stream. Furthermore, the cohesive nature of the silicone material enables the shape of your new breast to remain relatively intact even after the shell has ruptured.

Advantages and Disadvantages of Cohesive Gel Implants

Cohesive gel implants offer the following advantages over traditional silicone implants:
Cohesive gel implants are more easily able to impart their shape on the breast. Cohesive gel is not subject to folds and rippling the way traditional silicone implants are. While ruptures may still occur in cohesive gel implants, they should be much less frequent than in traditional silicone implants. Furthermore, in the event of a rupture, cohesive gel implants will maintain their shape better, and they are less likely to leak silicone into the blood stream. Cohesive gel implants are less likely to result in firm scar tissue, decreasing the chance of capsular contracture.
Cohesive gel implants may result in the following disadvantages compared with traditional silicone implants:
Since cohesive gel imparts its shape on the breast, it is crucial that the appropriate shape is chosen before the implant is inserted. There is more margin for error on implant shape with traditional silicone implants. Cohesive gel implants feel more firm than traditional silicone implants. Cohesive gel implants are significantly more expensive than traditional silicone implants. If a cohesive gel implant rotates, it is much more likely to result in a distorted breast shape.
Currently, there are only a few surgeons in the United States performing breast augmentation with cohesive gel implants. If you are interested in this option, make sure your plastic surgeon carefully explains the differences between cohesive gel and traditional silicone so that you can make a fully informed decision.

Frequently Asked Questions

  1. QUESTION:
Why do you have to be 22 to get silicone breast implants?
I've been reading that you have to be 22 to get silicone implants. I'm 18 and I'm planning on getting implants within the next year or two & i want the silicone ones. Is there anyway that I can get silicone implants even of I'm not 22 like have my mother or something sign something for me?

    • ANSWER:
The main reason that women are encouraged to wait until they're in their 20's to get breast implants of any kind (not just silicone) is because your body is not done growing yet. Many young women are still filling out well into their early 20's. If you get implants too early, they could end up having to be re-done if your body changes too much.

Wait the extra four years and have them done when you're 22. At your young age, even if your boobs aren't what you want them to be, you're still ahead of any woman in her 30's so try not to sweat it.

  2. QUESTION:
What's the difference between regular silicone and gummy bear silicone breast implants?
if there is any difference. also, is it rare for doctors to do silicone implants on patients younger than 22 if they have parents permission?

    • ANSWER:
Hayley has it backwards. Regular Silicone is the older style implant with liquid, that is not approved for cosmetic use. You can only choose that option if you had a mastectomy. The cohesive silicone gel implant is just that, you can cut the implant in half and the silicone will not leak. The have been used in Europe for years, but were just FDA approved in the US in 2006. They were only tested in clinical trials on women 22 and over, so those are the only women that doctors can use them with. Any doctor that does not follow these guidelines is sketchy and doesn't mind risking his license. It's an FDA standard, it has nothing to do with parental permission.

  3. QUESTION:
How would you know if your silicone implants are leaking?
How would you know if your silicone implants are leaking?

    What are the symptoms?

    • ANSWER:
Ruptures are not normally common, in fact most breast implants are rather durable. A study was done with 106 women who had breast implants for on average 11 years, only about 15% of them showed signs of a rupture. Another study also showed that only about 3% breasts have showed signs of deflation and leakage after 3 years.

A ruppture occurs when the shell of the implant develops a tear or a hole. Rupptures in silicone implants are usualy "silent" as there usualy are no signs of leaking. However, saline implants with deflate when they ruppture.

  4. QUESTION:
Are silicone or saline breast implants safer?
I know they both have health risks. Saline implants are coated in silicone I think. And silicone implants have a bad reputation when it comes to safety.

    • ANSWER:
the outter shell of both silicone and saline implants are made of silicone. the same silicone is used to make heart valves, catheters, dentistry implants, as a cosmetic filler and many other bodily devices.
true, if saline ruptures, the saline will absorb into your body and you will know that your implant has ruptured.
the newly FDA approved silicone implants rarely show evidence of rupture. my plastic surgeion showed me a silicone implant that he had puntured, when i squeezed it, the silicone squeezed out and when i quit squeezing, it sucked back in. the silicone is not a liquid, it is a cohesive gel. therefor it does not circulate thru your body if it ruptures. In fact, it is recommended that girls with silicone implants get a MRI of their breasts every 2-3 years to check for rupture. However, my plastic surgeon told me that as long as the silicone is really staying inside of the shell, there is no real reason replace the implant.
the long term safety cannot be established since the new silicone implants have only been on the market since 2006. there have been extensive and strenous studies on the new implants as mandated by the FDA.
in general, the long term safety cannot be assured or refuted as there is no long term evidence.

Sorry if this seems judgemental too you but seriously any one underage getting implants is just plain obscene and ridiculous. 15. QUESTION:
Fibroadenoma removal after breast augmentation. Will the removal of a Fibroadenoma affect my silicone implants?
I had breast surgery done about 9 months ago. Now I am going to get surgery to get a Fibroadenoma removed. It has been biopsied and it has been confirmed that it is Fibroadenoma and I want to get it removed. I am afraid that the removal will cause my implant to rupture or move. Should I be concern? My implants were done under the muscle.

    • ANSWER:
Not to worry I have had SEVERAL of those removed. Its not going to effect your implants at all since they were done under the muscle. They are going to make a small incision and most likely the lump will be pretty close to the surface as most Fibroadenoma's are. You might be in slight pain but everything will be ok.

  19. QUESTION:
Is it better to get the new silicone implants or saline breast implants?
Do you recommend getting the implant through the nipple, under the breast or thru the armpit?
Over or under the muscle? Thanks

    • ANSWER:
Silicone vs. Saline really depends on you.
Silicone is more realistic looking and recommended with girls whose natural breast size on is on the smaller size. For silicone you're going through the nipple no matter what. For saline you can go through the armpit or underneath the breast (with the armpit being the popular vote because of less obvious scarring) because the actual breast implant isn't filled until it's inside the breast and thus the incision does not need to be so big. Saline is recommended to woman who already have larger breasts because they're safer than silicone and since you already have so much breast tissue, the more natural feeling implant is not needed.
Under the muscle, it's more natural looking and feeling.
Another thing you have to think of with silicone is that since you do have to do through the nipple, you risk loosing feeling in that area as well as the possible loss in the ability to breastfeed future childeren(because milk ducts may be severed.)
One last thing is that if a breast implant is ruptured, it's immediatly obvious with saline, and with silicone you have to get an MRI in order to see the damage, so you could become very sick.

So it's really all up to you. You future plans, current breast size, desires, and risks you're willing to take!

What happens when a women have breast implants silicone and then she's having a baby?
what will happen to her breasts would they grow on top of the silicone or...... I dont know tell me what will happen. will the milk still come out her breast?

    • ANSWER:
the implants aren't put over the breast, they're actually either above or under the muscle in the chest. i believe you are still able to produce milk, the only concern is the shape of the breasts after you have the child. if you just got the surgery is isn't recommended to get pregnant within 2 yrs. it won't give your new breasts the proper time to heal.
      hope this helps!

  25. QUESTION:
What to get Silicone or Saline implants?
My wife is getting breast implants but we are not sure what is better to get. We save a lot of money if we get Saline implants but I still think the Silicone looks and feels more natural. And the maintence for silicone is expensive as well. For those who have either what do you think to get or how do you like yours?

    • ANSWER:
Saline is not dangerous at all. I don't know where that information came from. In fact, saline would be far safer in the event of a rupture. With saline, it is just salt water, the same fluid that's already in your body. It will just be absorbed by your body and eliminated like any other fluid. Silicone on the other hand will seep into the surrounding tissue and is usually pretty painful. If you catch the rupture early, it's not too difficult to remove, but if it leaks for an extended period of time, it can be pretty traumatic to remove all the silicone.

I'm sorry I can't answer your actual question, I really feel that the decision is best made between you, your wife, and her surgeon after weighing all the pros and cons. Depending on the look, cost, and maintenance you are wanting, her surgeon can make recommendations as to what would suit her best. I have been researching implants for about a year now, and if there's one thing I have learned, it's that no one thing works best for everyone.

  26. QUESTION:
Breast Implants.. I thought silicone was outlawed, but my friend just got some. ?
my friend just got some put in and they look great... do the saline implants look as good. What's the difference between silicone and saline and are the silicone implants still dangerous?

    • ANSWER:
      Hello,

Silicone gel implants have once gain been released by the FDA for cosmetic augmentation (with restrictions) in the US late last year. They are not dangerous per se, but do have more risk than saline-filled implants in that they tend to harden more over time. Saline look about as good as long as you do not go really large and you choose to have them placed underneath the Pectoral muscles.

      Best Regards,

      John Di Saia MD

  28. QUESTION:
What are the differences between silicone and saline breast implants?
What are the differences between silicone and saline breast implants?
    I thought they are same.

    • ANSWER:
Silicone went off the market for a while in the US for safety reasons. If the saline raptures it wont make you sick like the silicone, but with the new implants it's EXTREMELY unlikely they'll rupture so silicone is making a big come back. Silicone is said to feel and move more naturally than salines.

  31. QUESTION:
whats the Differance between Silicone and implants boob jobs?
I wanna have a breasts surgery but i wonder whats the differance between Silicone and implants?and whats better?anyone who knows anything about it pls help....iam not doing it these days but iam thinking about collecting information about it first..iam 17 by the way....thanx

    • ANSWER:
there are many types of 'boob jobs' some only countour the breast or raise it and some involve silicone implants. Most people refer to implants when speaking about a boob job. In order to increase your bust size the doctor must place a silicone filled sac into your breast. They are generally thought of as safe, however can interfere with breastfeeding later in life.

In Japan and Europe there is a new technique emerging that seems promising. It involved no implants but rather an injection of fat that includes fat producing adult stem cells (not from an embryo but from the fat in the body itself). This would make the body produce more fat in the breast area resulting in larger breasts as breasts are almost all fat. If the results are lasting (not enough information yet) this would be the best for any woman. There is no risk of rejection, no actual surgery and it will look and feel natural, as basically it pretty much is!

I would hold off a few years as your breasts will still grow. My breasts were fairly small until I was 22. Now they are perfect for my body. But also hold off so you can see how this new method pans out. It will be safer, cheaper, more effective and of course have better results.

  32. QUESTION:

  33. QUESTION:
Has anyone regretted getting silicone breast implants and why?
Thinking about having the surgery but worried about silent ruptures, pain, etc. Hopefully someone in the DFW area can help me decide whether or not to go through with this surgery and find a good doctor as well. I hear the Gummy Bear implants are safest.

    • ANSWER:
Plastic surgery should be for fixing superficial things that are grossly abnormal.

That being said, unless there is something hideously wrong with your breasts, going through surgery to get breast implants is a huge mistake. Most men prefer natural breasts and 9 times out of 10 breast implants do not look real without clothes and about 50% of the time it is easy to tell even if the girl is wearing clothes.

Besides, what kind of man (or woman) will you attract by being fake? Do you really want that kind of relationship? Not to mention there are plenty of women who regret getting them and eventually have them removed.

I highly suggest against getting them.

  34. QUESTION:

I want to make it clear that I trust their conclusion. However, many people still feel that the conclusion is false and that silicone breast implants are dangerous. Is there any legitimate scientific reason, that isnt some conspiracy, as to how the FDA might have come to an incorrect conclusion and if so what is it?

  40. QUESTION:
Are silicone implants heavier than real breasts?
Assuming both are of the same size.

  41. QUESTION:
Any professional opinions on the durability of silicone implants made recently vs. old ones from the 80s etc?
I understand that many problems with rupturing were in the valve itself, but they seem to be making them smaller? Any opinions and medical facts appreciated

    • ANSWER:
The new ones are very durable and will not leak.

  42. QUESTION:
are they planning on changing age restriction for silicone breast implants?
i know its because silicone has only been tested on 22 year olds, but are they considering testing younger like 18? i would think so...anyone know?

    • ANSWER:
Breast implants are considered medical devices, and they have to have FDA approval. Since they were only tested on women 22 years and older, that is the only population they were approved for. (you think a doctor would know that). Since they were approved by the FDA in 2006, I am sure they began testing them on women 18 and over. It will be some time though (years) before they are found to be safe. Unfortunately, this is confidential information, and we won't know anything about the studies until they are concluded.

  43. QUESTION:
How real can silicone breasts implants be?
I'm thinking to go for it since I'm really interested in having larger ones. But I just don't want my partner to find out that they are not my real breasts. So can it work out or seem real?

    • ANSWER:
If you have an excellent surgeon and only have a modest increase then it will seem natural. By excellent, I mean expensive. All those girls walking around with the baseballs on their chests went to the cheapest surgeon they could find and went at least two cup sizes bigger. If you have some natural breast tissue to mask the implant then this will make them seem natural. The more natural tissue you have, the bigger you can go but if you're an A cup with very little tissue then it will more than likely be noticeable. If you're a B and you increase to a C it probably would seem natural, but a B to a D, probably not.

On a side note, I think it's important for your partner to know if it's serious. I'm not judging, but a relationship based on dishonestly is unhealthy. Most guys don't care. Even the ones who say they do. I increased my size modestly by an experienced surgeon and no one can tell they're fake, so I've dated a couple guys who have said flat out that they would never date a girl with fake breasts but once I told them mine were fake, it didn't change anything. They're referring to the girls who LOOK like they have fake breasts because they went too big.

Just tell your experienced surgeon, your number one priority is looking natural and they'll make it happen. Best of luck to you!

  44. QUESTION:
What is the difference between saline and silicone breast implants?
What are the pros and cons of each. does it really matter? Which kind is "softer" if you were only going to go up to a full B or a small C. ?

Just Trying to do a little research for a possible future surgery. Thanks

    • ANSWER:
Some surgeons feel that silicone implants have a more natural look and feel than saline implants because silicone gel has a texture that is similar to breast tissue. Saline implants have some advantages over silicone implants. Silicone implant ruptures are harder to detect. When saline implants rupture, they deflate and the results are seen almost immediately. When silicone implants rupture, the breast often looks and feels the same because the silicone gel may leak into surrounding areas of the breast without a visible difference. Replacing a ruptured silicone gel implant is more difficult than repairing a saline implant. Silicone implants also have a higher rate of capsular contracture and a higher deflation rate.
Saline implants have a silicone rubber shell that is inflated to the desired size with sterile saline. Most implants have a valve that is sealable by the surgeon.

There are two types of saline-filled implants. One type is a fixed volume implant, which is filled with the entire volume of saline at implantation. Another type is an adjustable volume implant, which is filled intraoperatively and has the potential for further postoperative adjustment.

Silicone implants have a silicone rubber shell that is filled with a fixed amount of silicone gel. Each implant has a patch that covers the manufacturing port of the implant.

Silicone implants vary in shell surface (smooth/textured), shape, profile, volume, shell thickness, and number of shell lumens. Most silicone gel-filled implants are not adjustable.

  45. QUESTION:
What's the difference between Saline implants and Silicone gel implants?

    • ANSWER:
      Silicone v. Saline Implants

Some surgeons feel that silicone implants have a more natural look and feel than saline implants because silicone gel has a texture that is similar to breast tissue.

Saline implants have some advantages.

      SALINE
Saline implants have a silicone rubber shell that is inflated to the desired size with sterile saline. Most implants have a valve that is sealable by the surgeon. There are two types of saline-filled implants. One type is a fixed volume implant, which is filled with the entire volume of saline at implantation. Another type is an adjustable volume implant, which is filled intraoperatively and has the potential for further postoperative adjustment. On May 10, 2000, the FDA granted approval of saline-filled breast implants manufactured by Mentor Corporation and McGhan Medical.

      SILICON
Silicone implants have a silicone rubber shell that is filled with a fixed amount of silicone gel. Each implant has a patch that covers the manufacturing port of the implant. Silicone implants vary in shell surface - smooth or textured; shape, profile, volume, shell thickness, and number of shell lumens. Most silicone gel-filled implants are not adjustable.

In the early 1990's it was reported that silicone breast implants were responsible for connective tissue diseases in some women. After a comprehensive evaluation of the evidence for the Association of Silicon Breast Implants with human health conditions, the Institute of Medicine concluded in June that there is "no definitive evidence linking breast implants to cancer, neurological diseases, neurological problems or other systemic diseases."

The U.S. Department of Health and Human Services states:
"For some years controversy has existed over silicone implants used for breast augmentation or replacement after mastectomy. Adverse effects from their use have been widely reported in the popular press, with conflicting information often appearing in the medical literature. This controversy led the FDA first to ban any use of these implants and then to permit limited use, mainly as replacement after mastectomy.
Silicone is used not only in breast implants but also in implants located literally throughout every part of the body. It has been used: to construct heart valves and other cardiovascular prostheses; to fashion catheters which are used for purposes ranging from medication delivery to cardiac monitoring; in dentistry; as a facilitator for nerve regeneration; in ophthalmology; in the respiratory tract; as a prosthesis or ingredient in prostheses for many parts of the skeletal system; as a tissue expander; as a cosmetic agent for treatment of scars and wrinkles; and in many other applications."
